Arriving at our Office
It is our goal to meet your scheduled appointment time as closely as possible. Plan to arrive at our office 15 minutes prior to your scheduled appointment time in order to complete our registration process before your appointment. If you arrive late for your appointment, you may need to be rescheduled to a later date.
We ask that you refrain from wearing colognes, perfumes, or any other strong scents at the time of your appointment due to the sensitivity of some patients and staff members.
As per your insurance plan, any appropriate co-payments will be collected at our check-in window. If you do not have your co-payment, your appointment will need to be rescheduled to another day. For your convenience we accept cash, check, money orders, or credit/debit cards. We do not accept post-dated checks or American Express cards.
What to bring
In order for us to serve you, please complete the following paperwork and bring along with you to your appointment:
- Patient Registration Form
- HEALTH HISTORY
- MEDICATION LIST
- Notice Of Privacy Practices
- Sign Notice of Privacy Practice Acknowledgement
- You will also need to bring along all current insurance cards and appropriate co-payments.
- Medical Records Release (optional)
